Between March 3 and March 6, 2025, unauthorized actors accessed and possibly exfiltrated files from Palo Verde Hospital’s IT systems. PVH has since notified law enforcement and initiated a detailed review to determine whose information was involved.
What Information was Exposed?
The data compromised in the breach may include:
- Full name and contact details
- Social Security number
- Date of birth and demographic information
- Medical record and patient account numbers
- Diagnosis, treatment, and prescription information
- Dates of service and provider names
- Health insurance details
- For some, financial account and routing numbers
